A vendetta might occur when the protagonists killed a family member of a civilian or the leader of an outlaw gang. When a vendetta is started by a character, they will attempt to run away and might come back to take revenge, usually by ambushing the protagonists in a location or in the Enemies in the Brush Random Encounter. Most vendettas are randomized but there are specific ones that are triggered due to the protagonists' actions and choices during certain quests. Vendetta is a gameplay mechanic opposite of Friend for Life.

- Hideki Saito: if Jane Bell gives his Farmstead Deed to Galen Weeks.
- Shelby Cross: if Jane Bell failed to kill Cross during the Bounty Hunter Journey, and later protagonists recruited Jane, Cross might ambush the posse.
- Jane Bell: if Alonso Bell or Flora Albright is killed in front of her.
- Cl'erns Qui'g: if Cecilia Oscuro is killed in front of him.

- Surprisingly, Zombies can start vendettas if their living relatives are killed in front of them. This typically happens during a plague outbreak and at a farm where dead people were buried close to where their living family members live.
- It's possible to be ambushed by two characters with vendetta at the same time. Depending on their factions, they might end up fighting and killing each other.
