Canker is a Temple of the Ancients and also serves as a base of operation for Ruth Bough and her Coven.
Description[]
Canker consists of two main areas: the pig farm above ground which likely provides pig parts for Ruth's operation, and the temple underground which is where Ruth carries out her experiments and dark rituals.
Pigman Journey[]
When Cl'erns Qui'g arrives at Canker, if Shelby Cross is still alive by this point, he can be seen delivering to Ruth's Coven two captives, who are to be turned into Pigmen. There is no key to the cage so Cl'erns has to pick the lock in order to free them.

Trivia[]
- The grave of Ruth's father, Joe Culloch Bough, can be found on the cliff, on west side of the map. The gravestone reads "Here lies Joe Culloch Bough, who taught his only daughter to fear him. Spirits forgive you, for I won't."
- His grave contains a wedding band with the names Bough and Culloch engraved into it.
- There is also a Dreamer's Vervain flower next to the grave.
- On the opposite cliff, there appears to be a sacrificial altar, and next to it, a Silver Stubbs Double-Barrel shotgun.
- In Ruth's office in Canker, there is a blackboard with a number of names. Clarence Quigley's name is marked by a red circle.
- On another board in the same room, there are pictures of various individuals, possibly Ruth's targets. Pictures of Galen Weeks, Pain, and Alonso Bell can be found there.
